Why eight fields instead of three?

The bureau returns a report only for a precisely identified person. So it usually makes sense to run SSN + DOB first and then add the report for the found person — that is also cheaper: $3.50 instead of $4.00.

In what form does the result arrive?

As a ready PDF file in the request history of your dashboard. It stays there for 90 days.
